JOB TITLE:Certified Nursing Assistant (CNA)

DEPARTMENT:Nursing

POSITION SUMMARY:The CNA position provides quality, direct resident care in all activitiesof daily living including; dressing, grooming, bathing, feeding, toileting, transferring, walking and positioning. With direction from the Licensed professional Nurse in coordination with the interdisciplinary team, the CNA provides individualized physical, psychological and social support to residents while maintaining dignity and achieving their optimal level of functioning. The CNA interacts with family, visitors and other health care providers while implementing the plan of care for residents.

QUALIFICATIONS:Must be on the Wisconsin Nursing Assistant Registry. Must be able toperform essential functions of the position with or without reasonable accommodation. Must be able to take written and verbal instruction from supervisors. Must be able to communicate with residents visitors and staff. Must qualify for employment, after criminal background check, per guidelines of the Wisconsin Department of Human Services.

RESPONSIBILITY: To Provide Quality Care, as Identified in the Care Plan, WhilePerforming Daily Needs

Assists residents in dressing and undressing.

Assists with oral hygiene and care of teeth and dentures.

Assists with haircare, shaving, shampoo, combing.

Assists with bathing (tub, shower, whirlpool) and nail care (for non-diabetic residents).

Provides pericare after incontinence and rinses soiled linens.

Assists residents with transfers in and out of bed and into wheelchair with correct use of transfer belt.

Assists to reposition or provide bed mobility.

Provides passive range of motion.

Participates in care conference as requested by nurse and/or provides appropriate information to the care conference coordinator.

Reports to nurse any changes in resident’s condition immediately and as necessary. Further reports off to licensed nurse at end of shift.

RESPONSIBILITY: To Provide for Adequate Nutritional Intake for Residents

Assists residents to the dining area.

Feeds or assists to feed residents.

Records intake and output.

Passes ice water and offers fluids to residents.

Passes and assists residents with nourishment.

Passes and collects trays.

Serves supplemental nourishment as directed by nurse.

Is aware of dietary restrictions (i.e., calorie count, thickened liquids).

Monitors intake of food and record.

Wears gloves when handling food and passing snacks and juice carts.

RESPONSIBILITY: Promotes Individuality of Residents While Care is Guided by theResident Bill of Rights

Is knowledgeable and aware of importance of the Resident Bill of Rights.

Give autonomous decision making to resident whenever possible.

Articulates meaningful discussion with residents based on knowledge of the individual resident background.

RESPONSIBILITY:*To Assist Nurse With Special Procedures as Requested*

Collects urine and stool specimens.

Measures weight and height of residents upon admission and at least monthly thereafter.

Provides catheter care including care of drainage bag and measurement of urine output.

To implement and complete critical care sheets as designated by the licensed nurse.

Provides care of body after death.

Monitors and records vital signs.

Knows policy and participates in fire, tornado and evacuation drills.

RESPONSIBILITY: To Ensure Safe Environment for Residents and Staff and to ProtectSelf and Others from Blood Borne Pathogens

Receives report from nurse making written notes as necessary.

Correctly uses transfer belts for transferring and lifting residents.

Correctly uses Hoyer or mechanical lift following the proper procedure as designated by Sunny Ridge.

Appropriately and correctly applies and releases restraints.

Reports to nurse any incidents involving residents, visitors, staff or self.

Knows and follows the Standard Precaution Policies.

Appropriately wears protective equipment required for standard precautions (i.e., gloves, face shield, fluid resistant gown).

Prevents the spread of disease/infection by following proper infection control procedures, to include proper hand washing and disinfection/cleaning schedules.

Follows procedure to appropriately bag infectious linens and other infectious equipment and disposes of them.

Maintains safe clean environment in residents’ rooms and facility including wiping up spills immediately.

Understands the vitally important task to answer any and all call lights promptly and courteously.

Uses correct body mechanics at all times.

RESPONSIBILITY: Promotes and Establishes Healthy Relationships and UsesProfessional Conduct with Residents, Families, Visitors, Supervisors, Other Health Care Professionals and Co-workers

Arrives at work area on time and ready to begin work. Is able to prioritize and be organized and

Efficient when interacting and communication.

Strictly follows Policy on absenteeism, tardiness and call-ins as outlined in Personnel Policy Manual.

Follows dress code, wears name tag, and limits personal phone calls.

Is courteous and cooperative with residents, families, visitors, supervisors and co-workers.

Maintains resident confidentiality at all times. Understands the need for complete privacy of all health care data.

Promotes a positive and professional image both in the facility and in the community.

Appropriately refers residents, families, and visitors to licensed staff to answer questions.

Abides by and promotes the Continuous Quality Improvement program of the Rehabilitation Center.

RESPONSIBILITY: Maintain Current Levels of Education and Facilitate theImplementation of New Policies and Procedures with the Facility

Attends a minimum of 12 hours of continuing education per year pertinent to your department and designated by the ADON.

Notifies registry of name and address change.

Attends Nursing Assistant meetings in facility.

Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.

Participates in inspections and audits as designated.

Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs.

Maintains current knowledge of Wisconsin Department of Health regulations.

Maintains and applies knowledge of current policies and procedures as outlined by Sunny Ridge.

Provides written documentation of education received outside of facility.

Job Types: Full-time

Sign on bonus: 1/2 paid at 6 month anniversary, second 1/2 paid at 1 year anniversary.

Must have excellent attendance with no violations.

If employee voluntary quits within first year of employment, employee must return any amount given as a Sign on bonus to Sunny Ridge.
